What this form does
By filling out the form, you can:
- Change your Payee Name – for example, if your business name has changed or you need checks issued to a different legal payee.
- Request reissued checks – if you need prior checks voided and reissued to a new payee name.
- Update your mailing address – to ensure checks are sent to the correct location.
Once you submit the form, our team will update your account and check files accordingly. Future checks (and any reissued checks) will be processed based on the information you provide.

- Provide your updated Payee Name and/or mailing address, whatever you need to have updated OR Indicate that you are missing checks so that we can investigate.
- If you need past checks reissued, indicate that on the form.
- Submit the form.

Frequently Asked Questions
1. Can I update both my Payee Name and my Address at the same time?
Yes. The form allows you to update one or both at once.
2. How long will it take for updates to be processed?
Typically, updates are processed within a max of 2 business days. Any checks already in the mail may still arrive with incorrect information by nature of being in transit at the time of the request. If you requested reissued checks, please allow the 2 business days to update and process reissue request, and 7-10 business days for USPS delivery timeline.
3. Will I get confirmation once my information is updated?
Currently, we do not have a method for emailing a confirmation for every request, but if you have questions regarding closure, please reach out to support@healthytogether.zendesk.com for confirmation.
4. What if I only need to update my address?
You can use the same form to submit an address change without changing the Payee Name.
5. What happens to checks already issued under my old Payee Name?
If you can no longer deposit checks with your previous Payee Name, you can request that they be voided and reissued with your updated information in this same form.
6. Do I need to submit supporting documentation?
In some cases (such as a legal name change or business entity change), we may request additional documentation to verify the update. If required, we’ll contact you after your form submission.
