If you are applying for the Farmers Market Nutrition Program (FMNP) and have more than one eligible senior in your household, here’s what you need to know.
One Application Per Person
Currently, the Healthy Together application supports only one applicant per household at a time.
This means that if you are applying for more than one person (such as yourself and a spouse, or two parents), you will need to submit a separate application for each individual.
Important Information for Each Application
Even though you are submitting separate applications, you should still:
Report the total number of people in your household on each application
Calculate your gross household income based on that full number of people
This helps determine eligibility based on household size and income.
Who Gets Enrolled
The individual listed on the application is the one who will be enrolled in the program and issued benefits if they qualify.
If you are a caregiver filling out an application on behalf of someone else, be sure to enter that person’s name and information, not your own.
