This guide will help you log in to your Healthy Together account so you can apply for benefits, check your status, or manage your existing application. You can access Healthy Together through the mobile app or web browserâwhichever is easiest for you.
â Step 1: Download the App or Use the Web
You can log in through either method:

Download the Healthy Together app from the App Store or Google Play.
đ» Web App
Visit your programâs web portal: [Search for your state program online]
â Step 2: Create Your Healthy Together Account
Open the app or website and click âGet Started.â

You can register using either:
A mobile phone number, OR
An email address

Youâll receive a PIN code by text or email to verify your account.
PINs are valid for 10 minutes
If your code expires, just click âResend PINâ to get a new one
đ TIP: Be sure to use the same email or phone number every time you log in to make accessing your information easier.
â Step 3: Choose Your State
After youâve entered your PIN code, youâll be prompted to select your state.
Scroll through the list and choose [your state]
Click Continue to move on
â Step 4: Select Your Program
On the program dashboard, choose the option that matches your role.
If you are a Participant (e.g., an older adult), select:
âParticipant: [your program]â
This ensures you are taken through the correct steps for your program.
â Step 5: Apply or Connect to Your Account
Once you're on the program homepage, youâll see two options:
Apply â If this is your first time and you havenât submitted an application yet, click Apply to get started.
Connect to My Account â If you've already applied and want to check your status or update information, click Connect to My Account
âčïž To connect to an existing application, be sure to use the same email or phone number you used when you first applied.

Make sure your PIN code hasnât expired (they last 10 minutes)
Check that you're using the same phone or email each time
Double-check that you've selected the correct state and program
